We recommend completing the forms on line and printing them.

*certified birth certificate has a registrar’s raised, embossed, impressed or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar’s office, which must be within 1 year of your birth. Please note, some short (abstract) versions of birth certificates may not be acceptable for passport purposes.

Beginning April 1, 2011, all birth certificates must also include the full names of the applicant’s parent(s). For more information, please see New Requirement for all U.S. Birth Certificates.

The City Clerk’s Office is an authorized U. S. Passport Acceptance Facility. The office is located at 300 E. Chapman Ave. in the City of Orange. Passport application processing services are available Monday through Friday, 7:30 a.m. to 5:00 p.m., excluding holidays. Please note that City Hall is closed alternating Fridays. Please call the office at 714-744-5500 for assistance.

If you are a first-time applicant or your previous passport was issued more than 15 years ago, you must appear in person and complete a passport application (Form DS-11) using black or blue ink. Do not sign the application until requested to do so. It is mandatory that applicants, under the age of 16, appear with both parents or legal guardians. If one parent is absent, the applying parent must present a notarized consent document, or court document proving sole custody, or a death certificate. Applicants, 16 – 17 years old, must appear with one parent who can present acceptable picture identification as noted below.

SPECIAL NOTICE – CALIFORNIA BIRTH ABSTRACTS ARE NO LONGER ACCEPTABLE – PASSPORT APPLICANTS BORN IN CALIFORIA MUST SUBMIT LONG FORM PHOTOREPRODUCTION CERTIFIED COPIES OF BIRTH RECORDS

One Passport Photo – Photo must be in color, 2” x 2” in size with a white or off-white background, taken within the last six months. Photograph should be taken in normal street attire—Uniforms, or clothing that looks like a uniform, cannot be worn in the photo. The City of Orange provides passport photo services at $10.00 per applicant.
